RATIONALE
Nausea and vomiting are common complaints for not only the initial
EMS request, but also as a result of motion created by the emergency
vehicle during transport. Vomiting in the Pre-Hospital setting
can lead to many complications, such as airway compromise or
aspiration. It is responsive to drug therapy.
DRUG
Promethazine Hydrochloride
(Phenergan™) is a very effective anti-emetic that can
safely be administered pre-hospital. Drug class is
Phenothiazine. It is an antihistamine (H1 receptor
antagonist) that has the following actions: mild anticholinergic
activity, antiemetic, potentiation of analgesics. Onset is rapid
with IV administration and duration of effect is for 4-6 hours.
INDICATIONS
*ONLINE-
Vomiting and/or significant nausea
Motion Sickness
Medication side effect (e.g. opiate analgesic)
*Must receive
Online Medical Direction approval prior to administration of
anti-emetic.
CONTRAINDICATIONS
POSSIBLE COMPLICATIONS
INTERACTIONS
Caution with MAO Inhibitors, CNS depressants, Anticholinergics,
Alcohol.

PREPARATION/ADMIISTRATION
1)
Draw up calculated dose of Phenergan in syringe.
2)
Dilute in 3 - 5 cc of sterile saline solution.
Administer over 1 minute thru a well
flowing IV line. |
