Families & Children Medicaid
Medicaid provides health care for children under 19, pregnant women and for families that meet income limits. If you do not meet the income limits for Medicaid, you will be automatically evaluated for coverage by FAMIS.
FAMIS provides health care for uninsured* children under 19 and uninsured pregnant women whose income exceeds the limit for Medicaid but who meet all other eligibility criteria.
*Health insurance cannot have been dropped, without good cause, in the last four months before application.
How to apply:
You may apply at our office or we will mail an application to you. You may also follow the links below to complete an application at home and mail or bring it to the office.

Applications are processed within policy standards of 45 days. Pregnant women shall have their eligibility determined within 10 days provided all required information is received within that time.
Information required at application:
- Original birth certificates must be viewed and copied at your local Department of Social Services.
- Proof of household income for the month prior to application date. Additional information may be requested by the eligibility worker.
- Proof of identity (picture i.d.) for all applicants age 16 and older.
- Doctor's statement of pregnancy.
