Independent Living Assessment
The Franklin County Department of Social Services Independent Living Program utilizes a unique and youth centered independent living assessment that involves an interview, paper testing, and online testing. Youth are assessed starting at age 14 and then reassessed every 18 months. The independent living coordinator will contact you to set up your assessment!
Want to know if your skills are improving and it's not time to reassess with the agency? Feel free to take this free online assessment, just follow the instructions below. You can even have your caretaker assess your skills and compare results! Don't forget to share your results with your social worker!
Please read the instructions prior to going to the assessment web-site.
- Choose the youth assessment you have been asked to take and click the "Begin" button. This information should have been printed on your instruction card from FCDSS.
- Read the Terms and Conditions information page. When you are done, and if you agree, click on the "Yes, I Agree" button.
- At the Login Information page, type the information that FCDSS gave you on your instruction card for ID. If they didn't give you any information, ask them for it. When you are done, click on the "Next" button.
- Read the short instructions and click on the "Next" button to begin taking the assessment. Use the scroll bar on the right side of the pages to make sure you answer all the questions on each page.
- Answer the questions about yourself the best you can. When you reach the end, click on the "I'm Finished" button.
- Shortly after clicking on the "I'm Finished" button, the Score Report will "pop up" on your screen. Please print the Score Report so you talk about it with your parent, teacher or social worker. You can also save it on your computer.
Directions for Caregivers
- Choose the caregiver assessment that corresponds with your youth and click the "Begin" button. If you are unsure of which assessment to take, please follow the link provided that will help you select the appropriate assessment.
- Read the Terms and Conditions information page. After you are done, and if you agree, click on the "Yes, I Agree" button.
- Fill out the information the Login Information page. When you are done, click on the "Next" button.
- Read the brief instructions and click on the "Next" button to begin taking the assessment. Use the scroll bar on the right side of the pages to make sure you answer all the questions on each page. When you reach the end of the assessment, click on the "I'm Finished" button.
- Shortly after clicking on the "I'm Finished" button, the Score Report will "pop up" (it should take just a few moments). We strongly recommend printing the Score Report. It also can be saved on your computer.
